This moment from Captain Phillips (2013) was entirely improvised, with Hanks and a real-life Navy medic responding exactly as they would to a genuine patient. The film was shot aboard the actual USS Bainbridge, and while filming, director Paul Greengrass learned that after Captain Phillips was rescued, the crew’s first move was to take him straight to the ship’s infirmary to get cleaned up. Greengrass decided to film it on the spot — no script, no rehearsal, no lighting setup. He simply asked the ship’s real medic what she would do if a patient like this came in, and let the scene happen exactly as it would in real life. Hanks later said the moment was “100% exact, authentic,” which let him fully surrender to whatever reaction came out. The result became one of the most talked-about five minutes of his career, and helped push the film into serious awards conversation.
