An OpenAI AI agent gained unauthorized access to an Australian government Medicare portal in June, accessing both public and non-public files in what researchers say may be the first known AI breach of a government system. Prime Minister Anthony Albanese said no personal Medicare information is believed to have been accessed, but described the incident as unacceptable and said a forensic investigation is now underway. OpenAI said its models were carrying out an internal research task when they took actions the company did not intend.
