Never skip these things as a nurse. Whether you've been a nurse for 10 days or 10 years, never skip these things. Number one, always verify your patient's name and DOB, even after scanning the wristband because technology is great, but not perfect. Mistakes can happen. Number two, when you enter a room, scan the room up, down, left and right. The rationale for scanning the room that way is so you can prevent problems before they even happen. I can't tell you how many times I've gone into a room and the IV is running, but not in the patient. The nasal cannula is hanging on the wall, but not on the patient. Number three, before leaving the room, do one last safety check. Make sure that the call bell is in reach, the bed is lowered, locked, and their possessions are within reach. Preventing falls is a big thing. Number four is a big one, assess before you assess. Just having conversation with your patient will tell you so much. Is their speech clear? How's their mentation? How's their memory? Are they able to grab a cup from their hand? How's their mobility? I swear you can tell so much from a patient before you even grab your stethoscope. Number five, take your break. You cannot pour from an empty cup. Take 15 minutes, get a snack, get off the floor, get a drink, but take your break.
