Common myths in nursing. If you make a mistake, don't chart it. Let's talk about it. Failing to document an error is concealment, not protection. Mistakes happen, your integrity lies in how you handle and document them. Instead of, I gave the wrong med, you can say, wrong med administered, provider notified, vitals stable, new orders followed. Instead of charting subjectively like patient rude and uncooperative, you have to start documenting objectively, like patient raised voice and refused vitals, education provided on importance, patient verbalized understanding. Sometimes it's easy to forget that charting is actually a legal document and that one day you might actually have to go to court for it, and the only thing that you're going to have showing proof is your documentation. It has 20 chapters and it even comes with templates that you can use if you have smart phrases. If you want to improve your documentation and protect your license, I'm going to try to link this one down below, but they have been selling out pretty fast, so if you don't see the link anymore, that means they've already run out of stock.
