Hook
Okay, I'm going to begin the procedure. You're going to feel my hand on your penis and it might be a little bit cold, okay? Okay. So for a male, you begin at the tip and work your way around. I'm going to ensure that the sterile tweezers do not touch my non-sterile hand or the disposable bag. I'm going to use every cotton ball that they provide. Okay, we can now dispose of these tweezers. You can set this tray to the side. It is no longer sterile and I am going to step out of simulation to get my lubricant. Thank you and I'm now stepping back into testing situation. Okay, I'm going to be inserting the catheter now. If you could take a deep breath for me. Okay, and for the male, we go all the way to the bifurcation. We then take our non-dominant hand and hold while we insert the 10 cc's of fluid to inflate the balloon. Okay, we have inflated 10 cc's into the balloon. I can take my syringe and set it to the side for later. I'm now going to pull back for resistance until I meet resistance. Okay, I have met resistance, so I am done with the sterile procedure. I'm going to take off my sterile gloves. This is no longer a sterile field, so I can set them over here. Perform hand hygiene. I'm going to put on clean gloves and put on a leg strap on the patient to hold the catheter in place. Okay, I'm going to be putting on a leg strap. It's just going to hold the catheter in place so that it doesn't get in your way. And I'm going to take the red portion and put it right there. It is just the strongest part of the catheter. And doing this, I'm being mindful that I'm not tugging or hurting the patient and I'm also ensuring that the catheter is facing downward so that urine can flow downward. Then I'm going to take the catheter bag and hook it on the bed, ensuring that there are no dependent loops in the tube. Okay, so there are no dependent loops coming down and the urine is going to flow downward. So I can now clean up my area, closing up the garbage. This is going to be my garbage area now for testing purposes. I'm not going to lower the bed or put the side rails up, but in or take away the drape or the syringe, but in real life, I would. I'm just going to cover the patient up before I come back three days later. So how was that procedure for you? It was good. Good, I'm glad to hear that. So I would now document the amount, color, clarity, consistency, and odor of the urine. I would document that I inserted a 14 French catheter into my patient, inserting 10 cc's into the balloon, and that the the patient um tolerated the procedure well. Okay, so thank you. I'll see you when we do the removal, okay? Okay. Okay, so it is now three days later. I'm performing hand hygiene. Hello, my name is Courtney. I'm going to be your nurse today. I helped insert the Foley catheter three days ago and I'm back again. Can you tell me your full name? John Smith. And your date of birth? 1-2-97. Thank you. So I reviewed your chart and we actually have an order to remove the catheter now. Do you have any questions about that? No. No? Okay, it shouldn't be a painful procedure at all. We're just going to simply take it out and take off the leg strap and everything, okay? So we're going to go ahead and get started by draining the urine. So I'm going to go ahead and put clean gloves on for that. Come over here and grab my chucks pad and my cylinder. I'm going to lie the chuck's pad on the floor underneath it and the cylinder. So we will unhook this, place it over the cylinder, undo the alligator clip, ensuring that it's over the cylinder because it's going to come out right away, okay? It is all out. I'm going to clip the alligator clip back up and flip this back up there, okay? Then I'm going to take this, place it on the bed, and for purposes of testing, I'm just going to move this under the bed, okay? I will now perform hand hygiene as I was just working with urine and I'm going to put on another pair of clean gloves to do the removal process. I'm going to go ahead and lift up your gown. Okay, I'm going to remove the leg strap now, ensuring again that I'm not hurting him by tugging on it. We are all done with that. Okay, we will begin by deflating the balloon. In real life scenario, you would um passively deflate the balloon, but for purpose of testing, we're going to actively deflate it. So you're going to feel my hand on your penis. Just going to hold it in place while I deflate actively. So we inserted 10 cc's of fluid into the balloon, so we want to ensure that we get 10 cc's back out or we can do when we see bubbles. So I am noting some bubbles, so I'm just going to let the last little bit through. Okay, and then I'm going to carefully remove it by coiling it in my hand around my finger, ensuring that it does not fly up. Okay, and we have removed that. We can put all of this in here and dispose of it in our disposable area. I'm going to take off these gloves, perform hand hygiene. How was that for you? It was good. Yeah, a little bit easier than the insertion process. Yeah. Good, I'm glad. So at this time, I'm going to get the patient back into position, make them comfortable before I leave them. Cover them back up and I put one side rail up and lower the bed. At this time, I would document that I did remove a 14 French catheter. I removed 10 cc's from the balloon. My patient tolerated the procedure very well and the amount, color, clarity, consistency, and odor of the urine, okay? Thank you, sir. You're welcome. I will take off my gloves and perform hand hygiene. Thank you.
