Nobody documented it. No explanation from the resident. And here's the truth about how that happens: it's never one person deciding not to care. It's everybody assuming somebody else already did. That's how an undocumented bruise turns into an unexplained injury turns into a survey citation. Not one bad nurse. A chain of people who each assumed the next person had it covered. Veronica didn't assume. She assessed it, completed the incident report, and documented the change in condition, right then. Everybody's trained to spot what's wrong.
