You admitted a patient with decompensated HFrEF. Now what? Part 2 is the practical inpatient approach to HFrEF meds: how to decongest, when to initiate each of the 4 pillars of GDMT, what may prevent you from starting them, what to do when loop diuretics aren’t working, and what should be in place before discharge.
