He's gone towards the shed with his extinguisher and about ten feet out, it's exploded into his face. Are you alright? Well, I should be better, but... [Laughs] I'm in good hands. I got enough attention. Major burns can be notoriously difficult. You get burns that don't look too bad to start off with, but carry on burning in the same way as if you have cooked something on the grill. It is still cooking, even when you take it away from the grill. Where's the worst bit at the moment? Well, the scalp's painful. Okay. Scalp c Because severe burns can be deceptive, the deeper the burn, the less pain that you have. Probably on a scale of about 3 out of 10. 3 out of 10. The patient can feel safe, but they can change very, very quickly to barely breathing and unresponsive. How are you doing pain-wise? It's a bit like stinging nettles. You know, but the head seems about the worst, at the moment. Right. Okay. And obviously I don't like my hairstyle much either. You know, it's an interesting one... I can't see it but I can imagine what it's like. Just a little bit of a clump just to one side there. Hiya, I'm Say, one of the plastic registrars, looking after the burns side of things. Tell me if it's too painful, okay? You feel me touching you there? Reasonably well. That's very stiff there. You're not struggling with your speech yet? Not really, no. But my face is a bit stiff because that took a blast of the heat. But it doesn't feel burnt, so it's gone very stiff skin. You burned across the whole thickness of your skin. So, you're going to swell. And the full thickness band of the skin that is now fully burned, will not have the ability to expand. Yeah. And it's going to stop blood supply going into your hands. And to stop that from happening you need surgery. He needs to go to theatre fairly soon, so we need to get CEPODS ready, really. required THEATRE Time is a bit pressing to get that arm de-compressed. Yeah. Erm.. Just need to get, get this done. Patients can get quickly hypothermic cold, which they might not actually survive. So, we have it hot. You're wearing full surgical gown, gloves, hat, mask. Sweat is running down your back, your face. Operating in that environment has its challenges. Okay, are we ready with the first clean, so we need some Soapy Betadine, please. Antiseptic antiseptic soluttion Now, what I want you to do is grab the skin when it comes through. So, you need to grab it, sort of, centrally. It can look like barbaric surgery in some ways, but this is all done with compassion. It's surgery with a purpose. What's his core temperature now, please? 35.7. So, let's get his legs quickly wrapped. Okay, lovely. Temperature of the theatre? 34 Shall we turn that down? Okay. I think that looks absolutely fine... It was serious business. I knew I was pretty sick. I'm still breathing. I'm determined now, not to end up in a box this week. [Laughs]
